While standing on the second rung from the bottom of the ladder, the pipe broke off and struck the employee across the back knocking him to the concrete floor below. A maintenance employee repairing a metal entrance gate was moving a vertical fence post. He attached one end of a rope to the post and the other end to the bucket of a skid steer vehicle. He was standing between the post and the skid steer vehicle when the vehicle's operator lost control of the vehicle. The vehicle lurched forward and trapped the employee's head between the post and the bucket of the skid steer vehicle, breaking his eye socket and cheek bone.

An employee was dumping cardboard boxes into a recycling container using a forklift. When he tilted the forks to allow the boxes to slide off into the container, the pallet also slid into the container. While attempting to remove the pallet, the employee slipped and fell, hitting his face on the ledge of the container. He continued to fall to the floor, hitting his face again on the pavement. He broke two eye sockets, his nose, and a bone behind his upper lip.

Two employees were preparing to weld a 6 inch pipe. The anchor bolts that were holding the pipe came out of the concrete. The pipe fell and struck the ladder on which the employee was working. The employee was thrown 4 feet off the ladder to the concrete below striking his head. The employee suffered fractured ribs, a fractured eye socket, and minor bleeding to the brain.
